About the Case:

Seeking physiotherapist to manage a chronic, highly specific spine condition for a 51-year-old male patient. The case involves a 25-year history of mechanically reproducible back pain (mid-to-lower thoracic) that acts as a direct trigger for severe functional gastrointestinal symptoms (Somato-Visceral Reflex Syndrome).

Clinical Findings:

Acute Schmorl’s nodes (D7-D9) with endplate oedema.
Thoracic degenerative disc disease with bi-foraminal narrowing at D10-D12 (impinging splanchnic nerve outflow).
Lumbar degenerative disc disease (L4-S1) with radiculopathy and chronic paraspinal muscle spasm.
Thoracolumbar scoliosis with compensatory curves.

Required Expertise & Techniques:

The ideal candidate MUST have specialized training and proven experience in the following:
Maigne’s Technique: Targeted manual therapy focusing on the T10–L2 thoracolumbar junction.
McKenzie Method: Specifically focused on extension protocols to manage disc pathology and Schmorl’s node healing while avoiding prolonged flexion.
Diaphragmatic Release: To address abdomino-phrenic dyssynergia and bloating caused by kyphosis/scoliosis diaphragm distortion.
Autonomic/Visceral Physiotherapy: Understanding of the splanchnic nerve (T5-T12) mechanics and somato-visceral reflex.
Core stabilization and postural retraining.

Requirements:

Minimum 5-7+ years of clinical experience in complex spinal musculoskeletal (MSK) physiotherapy.
Background in managing referred neuropathic and visceral pain.
Willingness to collaborate on a 6–8 week structured program (3-5 sessions per week).
